Itisaproblemthatisoftenconfusedwithhomonymoushemianopsia.Hemispatialinattentionmayoccurwithorwithoutahomonymoushemianopsia.Whilehomonymoushemianopsiaisaphysicallossofvisualfieldtothesamesideinbotheyes,visualneglectisanattentionproblemtoonesideoftheirbody.Wecandividepatientsintothreegroups;thosewithahomonymoushemianopsiaandnohemispatialinattention;thosewithhemispatialinattentiononly(nohemianopsia)and,thosewithbothhomonymoushemianopsiaandhemispatialinattention.Itisimportanttodeterminethestatusoftheneglectversusfieldlosssothatpropertreatmentscanbeinitiated. Hemispatialinattentionismorethanavisualcondition,apatientwithhemispatialinattentionmaynotrespondtoavoicewhenthepersonisontheleftsideandmaynotbeabletorecallmemoriesofobjectsorplacesthatwouldnormallybetohisleftinaspecificsituation.Amanwithhemispatialinattentionmayonlyshaveonesideofhisfacewhileapatientwithonlyahemianopsiawouldcompensatebyturninghisheadandeyestoshavebothsidesofhisface.Theseproblemscanrelatetobraininjuryindifferentareas,butareoftenassociatedwithdamageintherightparietallobeofthebrain.Thepatientmayormaynothavealossofvisualfield,butduetothelefthemispatialin-attentioncannotlearntocompensatebecausetheycannotmentallyattendtoleftside. Visualneglectusuallyindicatesaworseoutcome.Overtimethenaturalcourseforvisualneglectistoslowlyimprove,butoutcomesarequitevariable.An18monthperiodhasbeensuggested,butresultsarevariable.Occupationaltherapymayimproveoutcomesinmanypatientsandvisualfieldexpandersmayhavepotentialtohelpinsomecases. Severehemispatialinattentioncanbesuspectedbygrossobservationalone.Thesubtleformscanbelessobvious,butmorecrucialforpatientswithmildneglectwhomaybeattemptingtofullyreintegrateintotheworldagainincludingdesirestodriveorreturntoworkwhicharesituationswhereevenmildhemi-spatialin-attentioncouldbedangerous.
